Document Request

OSM keeps student records on file for 25 years. Pursuant to Public Law 93-380, known as Family Education Rights and Privacy Act, Oregon School of Massage students have the right to review all official files and records pertaining to themselves. Picture ID is required to access student records. The student may challenge the content of a record they consider inaccurate, misleading or otherwise in violation of their privacy or other rights.

Transcripts

Official Transcripts

An Oregon School of Massage transcript will be released upon written request of the student. Please complete the Document Request Form and be sure to include the student's signature and social security number. The fee for Official transcripts is $5.00 per copy. Transcripts are processed within 48 hours of receipt of completed request.If you have any questions, please contact the Registar.

Unofficial Transcripts

A student may request an Unofficial Transcript at no charge. Unofficial transcripts do not contain the OSM seal and may not be sufficient in all situations. Unofficial transcripts may be faxed, include a fax number on the request form. Please use the same request Document Request Form.

Elective Class (CE) Completion Certificate

Students may request a duplicate copy of a certificate of completion from an elective class at a fee of $5.00 per certificate. Please use the same Document Request Form.
